Chronic Care Management & Remote Patient Monitoring

For patients with qualifying insurance coverage, we provide ongoing support for chronic conditions through remote monitoring technology and coordinated care management, helping you maintain better health outcomes between office visits.

These services complement our traditional primary care offerings, giving you access to cutting-edge medical technology and personalized care approaches that can help prevent illness and optimize your health.

  • Primary care internal medicine is a type of medical care for adults. These doctors, called internists, focus on preventing, diagnosing, and treating diseases in people 18 years and older.

Your internist is often your main doctor who:

    • Provides routine check-ups and physical exams

    • Manages ongoing health conditions like diabetes, high blood pressure, or heart disease

    • Treats common illnesses like infections, headaches, and stomach problems

    • Recommends vaccinations and health screenings

    • Coordinates your care with specialists when needed

Internists are trained to handle a wide range of adult health issues. They look at your whole body and overall health, not just one specific area. Think of them as your health care "home base" – they get to know you and your medical history so they can provide the best care for your individual needs.

Most adults see an internist for their regular medical care and only visit specialists when their internist refers them for more focused treatment.

  • MDVIP is a membership-based primary care program that gives you more personalized medical care. When you join MDVIP, you pay an annual membership fee to get extra services and benefits that regular insurance doesn't cover.

    Here's what makes MDVIP different:

    • Smaller patient groups: Your doctor sees fewer patients (around 600 instead of the usual 2,500-3,500), so you get more personal attention

    • Longer appointments: You'll have more time with your doctor during visits

    • Same-day or next-day appointments: It's easier to get in when you need care

    • 24/7 access: You can reach your doctor by phone or email outside normal hours

    • Annual wellness program: You get comprehensive health screenings and tests not usually covered by insurance

    • Focus on prevention: Your doctor spends more time helping you stay healthy and prevent problems

You still use your regular health insurance for medical treatments and sick visits. The MDVIP membership fee pays for the extra conveniences and wellness services. Most MDVIP doctors accept private insurance and Medicare for regular medical care.

  • Precision health is a modern approach to healthcare that treats you as a unique individual rather than using a "one-size-fits-all" method. This innovative approach tailors disease prevention and treatment by taking into account differences in people's genes, environments, and lifestyles.

Instead of giving everyone the same treatment for the same condition, precision health uses your personal information to create a healthcare plan that's specifically designed for you.

    How Does Precision Health Work?

Think of precision health like getting a custom-tailored suit instead of buying one off the rack. Your doctor considers several key factors about you:

Your Genetics: Your DNA holds important clues about how your body responds to different medications, your risk for certain diseases, and what treatments work best for you.

Your Environment: Where you live, work, and spend time can affect your health. This includes things like air quality, stress levels, and exposure to toxins.

Your Lifestyle: Your diet, exercise habits, sleep patterns, and daily activities all play a role in your health and how treatments might work.

Your Medical History: Your past health conditions, family history, and how you've responded to treatments before.

    What Are the Benefits?

Precision health offers several advantages over traditional healthcare:

    • Better Treatment Results: This approach allows doctors and researchers to predict more accurately which treatments will work for specific patients

    • Fewer Side Effects: By understanding how your body processes medications, doctors can choose drugs that are more likely to help you with fewer unwanted effects

    • Early Prevention: Genetic testing can identify your risk for certain diseases before symptoms appear, allowing for early prevention strategies

    • Personalized Care Plans: Your treatment plan is designed specifically for your unique needs and characteristics

Examples of Precision Health in Action

    • Pharmacogenomics: Testing how your genes affect your response to specific medications

    • Cancer Treatment: Using genetic information from tumors to choose the most effective cancer treatments

    • Heart Disease Prevention: Identifying genetic risk factors for heart problems and creating personalized prevention plans

    • Nutrition Planning: Using genetic information to understand which foods work best for your body

    The Future of Your Healthcare

Precision medicine is moving us closer to more precise, predictable and powerful healthcare that is customized for the individual patient. As technology advances, we can use more sophisticated tools to understand your unique health needs and provide better care.

This approach represents a shift from treating disease after it occurs to preventing it before it starts, using your individual characteristics to keep you as healthy as possible throughout your life.
